The rights provisioned under the General Data Protection Regulation can be exercised by submitting an application to the controller in person or by proxy. The written application may be submitted by post, through a licensed postal operator or electronically, signed with a qualified electronic signature. In the application you should indicate your name, address and other identification as a data subject, describe your request, inform on your preferred form of communication and actions on your request. You need to sign your request and provide the date of submission as well as your correspondence address.
If you believe that your rights under the General Data Protection Regulation have been violated, you have the right to file a complaint with the Data Protection Commission.
